Works in coordination with nursing leadership in an assigned unit to ensure work is accomplished effectively. The Patient Care Coordinator is the first line manager and is responsible for supervising, directing, coaching and mentoring. The Patient Care Coordinator collaborates with the Clinical Director on clinical and non- clinical issues. Responsible for the day-to-day operations of the clinical area by collaborating with the PCN, physicians and other health care practitioners.
Education and/or experience: Current licensure to practice professional nursing in
Tennessee. Competent in BLS and / or other specialized life support requirements designated
by work area or completion upon hire.
Minimum of three years practice in inpatient nursing preferred.
Knowledge: Knowledge of scope of the registered nurse, license practical nurse and nursing
assistants. Knowledge of and appropriate application of nursing process. Knowledge of
infection control procedures, safety precautions, techniques involved in administering routine
medications and / or treatments to patients. Basic computer knowledge.
Skills: Patient Assessment, Individualized Care planning, Critical thinking skills, Excellent
Customer service, Active listening
Abilities: Ability to communicate clearly with patients, families, visitors, healthcare team,
physicians, administrators, leadership and others. Ability to demonstrate emotional maturity by
establishing and maintaining effective working relationships. Ability to independently seek out
resources and work collaboratively. Ability to assess nursing needs of acute and chronically ill
patients and their families. Ability to teach patients and families in accordance with the nursing
care plan. Ability to use sensory and cognitive functions to process and prioritize information,
treatment and follow-up. Ability to use computer and learn new software programs. Ability to
document and communicate pertinent information using computer and / or paper
documentation tools. Ability to think critically and problem solve while remaining focused and
organized in a stressful and fast paced environment. Ability to work flexible hours and days to
meet facility needs.
